Admission Packets and Resident/Patient Guides
IlluminAge supports senior living and health care providers with custom admissions documents and resident/patient handbooks.
These client-specific guides bundle all or nearly all information a long term care facility or other health care provider gives to a new resident or patient and the person's family. They are neat and attractive, ready to use out of the box, less expensive than other alternatives, and easily published to CD-ROM or your web site.
Specifics include:
- These booklets are typically 36-48 pages in length and are developed as a custom publication for your organization or facility. They typically include provider-specific policies and procedures, as well as required information on: residents' or patients' rights and responsibilities; advance directives options under state law; and Medicare and Medicaid.
- These custom guides typically include one or more pull out inserts. For example, a second "signing" copy of your admission or intake agreement and your current rate schedule would typically be included as inserts to the package.
- The member price for setting up this publication is approximately $1,600-$1,800 on a one-time basis. Production is additional at about $3.60 per packet at a quantity of 500 and total publication length, including inserts, of 40 pages. Your cost will vary somewhat depending on length and complexity, but we will give you a firm price quote as your project gets underway.
- The contents of your custom resident or patient guide may also be published to CD-ROM and/or to your web site at a modest additional cost.
